Terms and Conditions
PAYMENT: $100 deposit (payable by check, money order, VISA, MasterCard, American Express or Discover) must be sent with the completed registration form. Balance is due by October 16, 2012.
FEE INCLUDES: All lectures, plenary session, continental breakfasts, refreshment breaks, banquet and Continuing Education Units.
FEE DOES NOT INCLUDE: Transportation to and from Chicago, Illinois, accommodations, meals (except where stated) and items of a strictly personal nature, such as phone and liquor.
REFUND POLICY: If registration is cancelled up to five business days prior to the start of the program, you will receive the full refund less $75 per person for administrative costs. If less than five business days, there will be no refund. Cancellation requests must be in writing.
